Job summary

Jobtailor is seeking a licensed Physical Therapist to evaluate patients in home health settings in Indiana. You will develop individualized treatment plans, provide direct PT services in patients’ homes, and monitor progress while collaborating with physicians and caregivers.

You will document findings and supervise therapy staff as needed. Requirements include an APTA-approved program, active PT license for Indiana, and at least one year of healthcare facility experience.

Qualifications

• Graduate of an APTA approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license or have reliable transportation to travel between patient homes and work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds including positioning or transferring patients and moving equipment
