DayToDay Health is seeking a licensed Physical Therapist to provide compassionate, evidence-based rehabilitative care to patients in Colorado homes. You will assess, plan, and modify treatments to improve mobility, safety, and quality of life.

In this role, you’ll educate patients and caregivers, document progress per regulatory standards, and collaborate with nurses and clinicians to deliver integrated care.

Qualifikationen

• Licensed/credentialed Physical Therapist with strong clinical judgment.
