Qualifications

• Colorado Physical Therapist license required.
• BS, MS, MPT, or DPT in Physical Therapy from an accredited program.
• Proficiency in patient evaluation and individualized care plan development.
• Experience with therapeutic exercise and balance, mobility training.
• Strong progress documentation and clinical record-keeping.

Responsibilities

• Conduct comprehensive patient evaluations in home health and assisted living settings.
• Develop and implement individualized physical therapy care plans.
• Provide therapeutic exercise programs tailored to patient needs and goals.
• Deliver strength, balance, and mobility training to reduce fall risk and improve function.
• Monitor and document patient progress accurately and timely.
• Adjust treatment plans based on patient response and changing clinical needs.

We are seeking a Home Health Physical Therapist to join our team serving the Denver area. This permanent position offers an opportunity to deliver individualized rehabilitation care to patients in home health and assisted living facility settings on a day shift schedule.

In this role, you will evaluate patients, develop individualized care plans, and provide therapeutic exercise and strength, balance, and mobility training to help patients achieve their functional goals and maintain independence. You will document patient progress thoroughly and collaborate with interdisciplinary teams to support safe, effective, and compassionate care across the Denver-area communities we serve, including Lakewood, Littleton, Golden, Centennial, and Englewood, Colorado.
