Physical Therapist – Home Health / Assisted Living

Position Overview

We are seeking a compassionate and motivated Physical Therapist to join a growing Home Health and Assisted Living team serving the Parker, Castle Rock, and Castle Pines communities. This role offers the opportunity to provide individualized, one-on-one patient care while helping patients regain mobility, independence, and overall quality of life in the comfort of their home or assisted living environment.

As a Physical Therapist, you will evaluate patient needs, develop personalized treatment plans, and implement evidence-based interventions focused on improving strength, balance, flexibility, coordination, and functional mobility. Key Responsibilities

• Evaluate patients and identify individualized goals and treatment needs
• Develop personalized care plans using evidence-based treatment techniques and interventions
• Provide skilled therapy services focused on improving mobility, strength, balance, flexibility, coordination, and overall function
• Help patients restore movement, reduce pain, and maximize independence and quality of life
• Educate patients and caregivers on exercises, wellness strategies, safety techniques, and injury prevention
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
• Collaborate with physicians, nurses, caregivers, and interdisciplinary healthcare professionals to coordinate care
• Maintain timely and accurate documentation of patient evaluations, treatments, and progress notes
• Promote healthy lifestyle habits and long-term wellness strategies for patients

Qualifications

Required

• Graduate of an accredited Physical Therapy program with a BS, MS, MPT, or DPT degree
• Current Colorado Physical Therapist license
• Strong clinical assessment and treatment skills
• Effective written, verbal, and interpersonal communication skills
• Ability to work independently while collaborating within a multidisciplinary team

Preferred

• Minimum of one year of Physical Therapy experience
• Home Health or Assisted Living experience preferred, but not required
