Health Care Administrators Association® (HCAA) seeks a Licensed Physical Therapist to provide home-based rehabilitation. You will assess patients in their homes, create individualized plans, and adapt interventions to enhance mobility, safety, and quality of life.

Collaborating with nurses, social workers, and other clinicians, you will educate patients and caregivers on goals and self-management, while maintaining thorough documentation in line with regulatory standards.

Qualifications

• Licensed Physical Therapist with strong clinical assessment and treatment-planning skills.
• Ability to deliver patient-centered, evidence-based interventions in a home setting.
• Effective communication and education skills for patients and caregivers.
• Strong documentation skills and attention to regulatory requirements.
