Calqulate Metrics Ltd is seeking a dedicated Physical Therapist to provide home health services across Colorado. This full-time role focuses on patient-centered rehabilitation delivered in patients' homes, with collaboration from a multidisciplinary team.

You will perform assessments, design tailored plans, guide therapeutic exercises, and educate patients and families about recovery and prevention. CPR certification and state licensure are required, with CAPTE-accredited education and home

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• CAPTE-accredited degree in physical therapy.
• Licensed to practice as a PT in the state of operation.
• CPR certification required.

Responsibilities

• Must be a licensed driver who can travel to all business locations
