Job summary

Jobtailor is seeking a licensed Physical Therapist to provide comprehensive home health services in the West Houston area. You will evaluate patients, develop individualized treatment plans, and deliver PT care in patients' homes.

You will monitor progress, document meticulously, and collaborate with physicians and caregivers as part of an interdisciplinary team. Travel between patient homes and sites is required.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Valid driver's license, acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
