BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time position performing home health visits in Vermont. This office services adult and geriatric clients across Randolph, Topsham, and Chelsea.

One year of clinical experience as a licensed Physical Therapist is required. As a home care PT, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home. The retention bonus applies to full-time positions.

Responsibilities include:

• Making home visits in designated territories.
• Performing diagnostic tests and measurements.
• Developing and implementing individualized care plans.
• Assessing and revising care plans, collaborating with nursing staff.
• Educating clients and families on rehabilitative activities.
• Documenting observations and interventions using electronic tablets.

Qualifications:

• Vermont PT license.
• At least one year of recent PT experience.
• Graduation from an accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
