Job Title: Physical Therapist (PT) - Home Health

Position: Full-time Physical Therapist performing home health visits

Overview

BAYADA is seeking an experienced Physical Therapist to join our team, providing rehabilitative care and skilled nursing to adult and geriatric clients in their homes. The role involves making visits within designated territories, developing care plans, and educating clients and families.

Responsibilities

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, including mobility, strength, balance, and safety assessments.
3. Develop and implement individualized care plans with manual therapeutic exercises, gait training, and other interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff.
5. Educate clients and families on rehabilitative activities and safety.
6. Accurately document observations, interventions, and evaluations using electronic medical records.

Qualifications

• Current New Hampshire Physical Therapist license.
• At least one year of recent clinical experience as a PT.
• Graduation from an accredited program by the American Physical Therapy Association or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
