BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full-time opportunity performing home health visits for our Vermont office. This office services adult and geriatric clients on a per-visit basis in Randolph, Topsham, and Chelsea, Vermont.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to remain at home. The retention bonus applies to full-time positions.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assess and revise the care plan regularly, collaborating with nursing staff as appropriate.
5. Educate clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

1. Current Vermont Physical Therapist - PT license.
2. At least one year of recent work experience as a Physical Therapist.
3. Graduation from an APTA-approved program or an accredited allied health program, evidenced by transcript or diploma.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Solid computer skills; prior EMR experience preferred.
