Job summary

Benchmark Therapy and Rehab in Dayton, OH seeks a compassionate Physical Therapist for home care. You will evaluate patients, develop plans, and use exercises, manual therapy and equipment to reduce pain and improve mobility.

Collaborate with physicians and case managers; maintain progress notes and communicate patient information to the care team. Requires Ohio PT license; full-time with a benefits package and mileage reimbursement.

Qualifications

• Licensed Physical Therapist required to practice in Ohio.
• Ability to evaluate and plan treatment for home care patients.
• Collaborate with physicians and case managers to coordinate care.

Responsibilities

• Evaluates and provides initial treatment with patients in home care.
• Creates treatment plans using exercises, stretching, hands-on therapy and equipment.
• Works with physicians, case managers and other therapists.
• Monitors and reports patient progress to the health care team.
• Requires state license to practice physical therapy.
• Reports to the manager of clinical services.

Position Summary

Physical Therapist- Home Care Full Time

Requirements And Responsibilities

• Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.
• Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
• Works cooperatively with physicians, case managers and treating therapists.
• Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
• Requires state license to practice physical therapy.
• Reports to manager of clinical services.
