Job summary

Professional Therapy Solutions is seeking a dedicated Physical Therapist to provide home health services in Kentucky. The role focuses on evaluating patients recovering from injury, disease, or surgery and delivering individualized treatment plans to improve mobility and reduce pain.

The position requires state licensure and a commitment to one-on-one patient care, with mileage reimbursement and flexible scheduling.

Qualifications

• Licensed physical therapist eligible to practice in Kentucky.
• Experience delivering home health physical therapy is preferred.
• Ability to develop and implement patient-specific treatment plans.

Responsibilities

• Evaluates and treats patients in a home care setting.
• Establishes patient treatment plans using exercises, stretching, hands-on therapy, and equipment.
• Collaborates with physicians, case managers, and other therapists.
• Monitors and documents patient progress and shares information with the care team.
• Reports to the manager of clinical services.

Physical Therapist- Home Care - Full Time

Requirements and Responsibilities:

• Evaluates and treats patients recovering from injury, disease, or surgery in a home care environment.
• Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands‑on therapy, and equipment to ease patients' pain and help them increase their mobility.
• Works cooperatively with physicians, case managers and treating therapists.
  
• Monitors, documents, and reports patients' progressand communicates needed health information regarding the patient to all appropriate health care team members.
  
• Requires state license to practice physical therapy.
• Reports to manager of clinical services.
