Job summary

Benchmark Therapy and Rehab is seeking compassionate Physical Therapists to join our home health team for full-time positions in the Dayton area. The role focuses on evaluating patients, establishing treatment plans, and collaborating with a multidisciplinary health care team to improve mobility and function in home settings.

Candidates must hold a valid state license to practice physical therapy and be able to work with an experienced team, with flexible scheduling and comprehensive benefits

Qualifications

• Valid state license to practice physical therapy.
• Experience in home health or home care settings preferred.
• Ability to develop and implement treatment plans.
• Strong communication with physicians and care team.

Responsibilities

• Evaluate patients and provide initial treatment in home care.
• Develop and implement personalized treatment plans.

• Monitor and document patient progress and share updates.
• Report to the manager of clinical services.

Position Summary:

Physical Therapist- Home Care Full Time

Requirements and Responsibilities:

• Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.
• Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
• Works cooperatively with physicians, case managers and treating therapists.
• Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
• Requires state license to practice physical therapy.
• Reports to manager of clinical services.
