Job summary

Cherrydale Post Acute in Greenville, SC is seeking a Physical Therapist to evaluate and treat residents, improving mobility and independence. You will collaborate with physicians, families, and a multidisciplinary team to deliver patient-centered care, with supervision of PTAs and staff.

Qualifications

• Degree in Physical Therapy required (Bachelor’s; advanced degree preferred).
• Active SC Physical Therapist license required.
• Continuing education and good standing with State Board required.
• Experience in a skilled nursing or rehab setting preferred.
• Familiarity with PCC and Casamba preferred.
• Strong clinical, communication, and leadership skills.

Responsibilities

• Perform patient evaluations within 24 hours of referral.
• Develop and implement individualized treatment plans.
• Provide therapy services to improve strength, balance, mobility, and function.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant clinical documentation.
• Participate in care planning, rehab conferences, and discharge planning.
• Educate residents, families, and staff on therapy programs and goals.
• Recommend and assist with durable medical equipment (DME) needs.
• Supervise PTAs, aides, and students.
