Job summary

Cherrydale Post Acute in Greenville, SC, is seeking a Physical Therapist to evaluate and treat residents, improving mobility, strength and overall function. You will collaborate with an interdisciplinary team to deliver personalized, patient-centered care in a supportive rehab environment.

Responsibilities include developing treatment plans, monitoring progress, documenting care, and participating in discharge planning. PRN scheduling offers flexible opportunities within our growing network.

Qualifications

• Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred).
• Active, unencumbered Physical Therapist license in South Carolina (required).
• Experience in a skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare (PCC) and Casamba preferred.
• Strong clinical, communication, and leadership skills.
• Commitment to patient-centered care.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans
• Provide therapy to improve mobility, strength, balance, and functional independence
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate and timely clinical documentation
• Communicate effectively with physicians, residents, families, and care team members
• Participate in care planning and discharge planning
• Conduct staff education and in‑service training
• Supervise PTAs, aides, and students as needed
