Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our Meadow View Health Services location in Sheboygan, WI. We offer a full-time daytime schedule and a comprehensive benefits package.

Applicants should hold a Doctor of Physical Therapy, a current Wisconsin PT license (NPTE passed), and prefer at least 1 year of PT experience to support patient evaluation, plan development, and progress documentation.

Qualifications

• DPT required.
• Current, unencumbered state PT license; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes

Physical Therapist Requirements & Responsibilities:
