Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our clinician-led team at Riverview Pointe in Olmstead Falls, OH. The role focuses on evaluating, developing, and implementing individualized treatment plans in coordination with other disciplines.

The successful candidate will hold a Doctor of Physical Therapy (DPT), possess an active state license (NPTE) and ideally have 1 year of PT experience.

Qualifications

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
