Continuum Therapy Partners is seeking a Physical Therapist to join its expanding team across Milwaukee, WI locations. The role focuses on evaluating and treating patients with DPT-qualified leadership and collaboration with interdisciplinary teams.

Candidates should hold a Doctor of Physical Therapy, a current WI license, and ideally 1 year of PT experience. Full-time daytime schedule with comprehensive benefits and licensure reimbursement offered.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE).
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
