Job summary

Sonoma Post Acute is seeking a Physical Therapist Assistant to support the Staff Physical Therapist in delivering high-quality resident care. The role involves direct patient interaction, documenting outcomes, and maintaining a safe treatment environment in accordance with regulatory standards.

Qualifications

• Associate degree required from PTA program.
• Ability to read technical procedures and policy manuals.
• Proficiency in documenting within health care systems.

Responsibilities

• Treat residents as directed by Physical Therapist.
• Complete timely, accurate documentation in accordance with facility and payer requirements.
• Maintain a safe treatment environment following infection control and safety protocols.
• Educate residents, families, and caregivers on treatment goals and home programs.
• Communicate progress and discharge plans with the team and physicians.
