Job summary

Adelphi Nursing & Rehab Center is seeking a dedicated Physical Therapist to join our full-time team. You will provide direct patient care, develop treatment plans, document progress, and collaborate with a multidisciplinary staff to promote resident well-being in a long-term care setting.

Qualifications

• Document patient condition and educate patients and families in PT methods.

Duties and Responsibilities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
