Job summary

Texas Counseling Association in Texas is seeking a dedicated Full-Time Physical Therapist (PT) to provide person-centered care in-house, helping residents improve mobility, strength, and overall quality of life.

You will assess conditions, develop individualized plans, and administer treatments while collaborating with an interdisciplinary team. Texas PT license and SNF experience are preferred; Net Health knowledge is a plus. Competitive benefits and flexible schedules are offered.

Qualifications

• Active Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Experience with Net Health EDS preferred.
• Strong communication and collaboration skills.
• Ability to make sound clinical decisions in a team setting.

Responsibilities

• Consult with residents to assess mobility and functional limitations.
• Evaluate assessments and test results to guide treatment.
• Develop individualized treatment plans with diverse therapy techniques.
• Create rehab programs tailored to patient goals and progress.
• Administer prescribed PT treatments to relieve pain and restore function.
• Deliver care according to the plan of care and regulatory requirements.
• Monitor patient responses and adjust approaches with the care team.
• Provide education and support to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and on time per policies.
• Attend meetings as designated by the Director of Rehab.

Join Our Team as a Full-Time Physical Therapist (PT)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in‑house team of outstanding professionals. In this role, you will provide person‑centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)

• Consult with patients to assess their physical condition and functional limitations

• Evaluate and interpret patient assessments and test results

• Develop individualized treatment plans using a variety of therapy techniques

• Create personalized rehabilitation programs tailored to each patient’s goals

• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing

• Ensure all treatments are delivered in accordance with the established plan of care

• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team

• Provide clinical support and education to patients, family members, and caregivers

• Collaborate with interdisciplinary team members to ensure high‑quality patient care

• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations

• Comply with all applicable rules, regulations, and procedures

• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Physical Therapist license

• In good standing with all regulatory agencies and licensing boards

• Skilled Nursing Facility (SNF) or long‑term care experience preferred

• Current knowledge of physical therapy treatment practices

• Experience with Net Health Electronic Documentation System preferred

• Ability to manage patients with varying personalities and needs

• Strong communication, collaboration, and clinical decision‑making skills
