Job summary

Northparkhealthcenter in McKinney, TX is seeking a full-time Physical Therapist (PT) to help residents improve mobility and quality of life within an in-house rehab team. The ideal candidate holds a Texas PT license, has SNF or long-term care exposure, and is proficient with Net Health documentation.

You will collaborate with a multidisciplinary team and provide patient-centered care according to the plan of care.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health documentation is a plus.
• Ability to manage patients with varying needs.
• Strong communication and collaboration skills.

Responsibilities

• Consult with patients to assess functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans and rehabilitation programs.
• Administer PT treatments per plan of care.
• Monitor patient response and adjust approaches with the rehab team.
• Provide education to patients, families, and caregivers.
• Document thoroughly and timely per policies and regulations.
• Attend designated meetings with the Director of Rehab.

Join Our Team as a Full-Time Physical Therapist (PT)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in‑house team of outstanding professionals. In this role, you will provide person‑centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)

• Consult with patients to assess their physical condition and functional limitations

• Evaluate and interpret patient assessments and test results

• Develop individualized treatment plans using a variety of therapy techniques

• Create personalized rehabilitation programs tailored to each patient’s goals

• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing

• Ensure all treatments are delivered in accordance with the established plan of care

• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team

• Provide clinical support and education to patients, family members, and caregivers

• Collaborate with interdisciplinary team members to ensure high‑quality patient care

• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations

• Comply with all applicable rules, regulations, and procedures

• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit
