Job summary

Kemp Care Center in Texas seeks a dedicated Physical Therapist (PT) to join our in-house team. You will assess residents, develop individualized treatment plans, and provide rehabilitation to improve mobility and quality of life.

We welcome new graduates; SNF or long-term care experience preferred. Full-time or PRN positions with competitive pay and benefits, ongoing professional support, and collaboration with an interdisciplinary team.

Qualifications

• Active Texas PT license in good standing.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret assessments and test results.
• Develop and implement individualized treatment plans.
• Create rehabilitation programs tailored to patients' goals.
• Administer physical therapy treatments to relieve pain and restore function.
• Document thoroughly per policies and regulations.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Attend required Rehab Director meetings.
• Monitor patient response and adjust interventions as needed.
• Provide education to patients, families, and caregivers.

Join Our Team as a Full-Time or PRN Physical Therapist (PT)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)

• Consult with patients to assess their physical condition and functional limitations

• Evaluate and interpret patient assessments and test results

• Develop individualized treatment plans using a variety of therapy techniques

• Create personalized rehabilitation programs tailored to each patient’s goals

• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing

• Ensure all treatments are delivered in accordance with the established plan of care

• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team

• Provide clinical support and education to patients, family members, and caregivers

• Collaborate with interdisciplinary team members to ensure high-quality patient care

• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations

• Comply with all applicable rules, regulations, and procedures

• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Physical Therapist license

• In good standing with all regulatory agencies and licensing boards

• Skilled Nursing Facility (SNF) or long-term care experience preferred

• Current knowledge of physical therapy treatment practices

• Experience with Net Health Electronic Documentation System preferred

• Ability to manage patients with varying personalities and needs

• Strong communication, collaboration, and clinical decision-making skills
