Creative Solutions in Healthcare - East is seeking a dedicated Physical Therapist (PT) to join our in-house team at a Texas facility. You will provide person-centered care to help residents improve mobility, strength, and overall quality of life.

The role offers full-time or PRN options with competitive compensation and comprehensive benefits. New graduates are welcome to apply; licensing and compliance with state regulations are essential.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations
• Evaluate and interpret patient assessments and test results
• Develop individualized treatment plans using a variety of therapy techniques
• Create personalized rehabilitation programs tailored to each patient’s goals
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical support and education to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Attend required meetings as designated by the Director of Rehab

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)

• Experience with Net Health Electronic Documentation System preferred
