Job summary

Hale Nani Rehabilitation & Nursing Care Center in Honolulu, HI, seeks licensed Physical Therapists to evaluate residents, design individualized treatment plans and guide rehab progress. Role emphasizes collaboration with nursing and interdisciplinary teams to restore independence and improve quality of life.

Qualifications

• Licensed in Hawaii or graduate of an approved Physical Therapist program or actively working toward licensure.
• Graduate of a CAPTE Accredited Physical Therapy Education Program.
• Experience in post acute care or similar healthcare environment preferred.

Responsibilities

• Patient evaluation, exercise instruction, and pain management.
• Develop individualized treatment plans to help residents achieve their goals.
• Provide equipment training and patient/family education.
• Document patient progress accurately and on time.
• Collaborate daily with nursing, therapy, and interdisciplinary care teams.
• Support residents in regaining independence and quality of life.
