Job summary

Qualifications

• Licensed in Hawaii or graduate of an approved Physical Therapist program or actively working toward licensure.
• Graduate of a CAPTE Accredited Physical Therapy Education Program.
• Experience in post acute care or similar healthcare environment preferred.

Responsibilities

• Patient evaluation, exercise instruction, and pain management.
• Develop individualized treatment plans to help residents achieve their goals.
• Provide equipment training and patient/family education.
• Document patient progress accurately and on time.
• Collaborate daily with nursing, therapy, and interdisciplinary care teams.
• Support residents in regaining independence and quality of life.
