Job summary

Ahava Therapy Services is seeking a Physical Therapist to support residents at Peachtree Centre in Gaffney, SC. The role offers full-time, part-time, and PRN opportunities within a collaborative rehab team.

You will evaluate residents, develop individualized plans, and coordinate with nursing and families to maximize function and quality of life. A current SC PT license and BLS/CPR are required; ongoing CEUs are encouraged.

Qualifications

• Active PT license in the state of practice (SC).
• BLS/CPR certification either current or obtained during orientation.
• Strong communication, teamwork, and organizational skills.

Responsibilities

• Review resident information and assess its impact on treatment.
• Conduct functional assessments and document progress.
• Develop and implement individualized treatment plans with measurable goals.
• Instruct and supervise therapy assistants as needed and document sessions.
• Collaborate with nursing staff and families to maximize functional abilities.
• Maintain timely records and discharge summaries.
• Ensure compliance with regulatory and facility standards.

About the Facility

Peachtree Centre is a 151-bed skilled nursing facility with 40 short-term rehab beds.

Responsibilities

• Review resident information and evaluate its impact on assessment and treatment.

• Conduct and document thorough functional assessments using standardized procedures.

• Develop and implement individualized treatment plans with achievable goals.

• Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.

• Instruct and supervise therapy assistants as needed.

• Enter treatment and progress notes for each session.

• Revise treatment plans as needed throughout the course of care.

• Document treatment in clear, concise discharge summaries.

• Orient residents to services and treatment procedures appropriate to their needs.

• Collaborate with nursing staff, residents, and families to maximize functional abilities.

• Educate residents and caregivers on the continuation of home exercise and therapy.

• Assist with referrals for additional services as needed and maintain knowledge of community resources.

• Participate in rehabilitation team conferences and other facility meetings.

• Access continuing education opportunities to maintain clinical competency.

• Participate in quality improvement initiatives.

• Follow regulatory, professional, and facility standards.

• Perform other duties as assigned to provide quality care.

• Understand payer reimbursement and allocate therapy resources efficiently.

• Maintain expected productivity levels.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy

• Current PT license in the state of practice

• BLS/CPR certification (current or obtained during orientation)

• Strong communication, teamwork, and organizational skills
