Job summary

Ahava Therapy Services is hiring a Full-Time Physical Therapist to join our in-house rehabilitation team at a 173-bed skilled nursing facility in Kingsport, TN. You will evaluate residents, develop individualized treatment plans, and document progress while supervising therapy assistants and collaborating with nursing staff.

Requirements include a Bachelor’s or Master’s degree in Physical Therapy, current PT license in the state, and BLS/CPR certification.

  • Organizational skills to maintain accurate records and schedules.
  • Current PT license in the state of practice and BLS/CPR certification.

Responsibilities

  • Review resident information and assess impact on treatment.
  • Perform and document functional assessments using standard methods.
  • Create and implement individualized treatment plans with clear goals.
  • Follow guidelines for evaluation, treatment, and documentation; keep records current.
  • Supervise therapy assistants as needed and provide guidance.
  • Document sessions with treatment and progress notes.
  • Adjust treatment plans as care progresses.
  • Prepare concise discharge summaries for residents.
  • Orient residents to services and procedures appropriate to their needs.
  • Collaborate with nursing staff, residents, and families to maximize function.
  • Educate residents and caregivers on home exercise and therapy continuation.
  • Refer for additional services and stay aware of community resources.
  • Participate in rehab team conferences and facility meetings.
  • Pursue continuing education to maintain competency.
  • Engage in quality improvement initiatives and follow standards.
  • Contribute to a professional image and maintain productivity.

About the Facility

Ahava Therapy Services is seeking a Full-Time Physical Therapist to join our in-house rehabilitation team at Wexford House, a 173-bed skilled nursing facility offering short- and long-term rehabilitation.

Responsibilities
  • Review resident information and evaluate its impact on assessment and treatment.

  • Conduct and document thorough functional assessments using standardized procedures.

  • Develop and implement individualized treatment plans with achievable goals.

  • Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.

  • Instruct and supervise therapy assistants as needed.

  • Enter treatment and progress notes for each session.

  • Revise treatment plans as needed throughout the course of care.

  • Document treatment in clear, concise discharge summaries.

  • Orient residents to services and treatment procedures appropriate to their needs.

  • Collaborate with nursing staff, residents, and families to maximize functional abilities.

  • Educate residents and caregivers on the continuation of home exercise and therapy.

  • Assist with referrals for additional services as needed and maintain knowledge of community resources.

  • Participate in rehabilitation team conferences and other facility meetings.

  • Access continuing education opportunities to maintain clinical competency.

  • Participate in quality improvement initiatives.

  • Follow regulatory, professional, and facility standards.

  • Perform other duties as assigned to provide quality care.

  • Understand payer reimbursement and allocate therapy resources efficiently.

  • Support Ahava’s mission and project a professional image.

  • Maintain expected productivity levels.

Qualifications
  • Bachelor’s or Master’s degree in Physical Therapy

  • Current PT license in the state of practice

  • BLS/CPR certification (current or obtained during orientation)

  • Strong communication, teamwork, and organizational skills
