Job summary

Ahava Therapy Services in Cordova, TN is seeking a qualified Physical Therapist for full-time and part-time positions. You will assess residents, develop treatment plans, and collaborate with an interdisciplinary team to maximize functional outcomes.

Requirements include a Bachelor’s or Master’s degree in Physical Therapy, a current TN PT license, BLS/CPR certification, and strong communication and teamwork skills.

Responsibilities

  • Review resident information and evaluate its impact on assessment and treatment.
  • Conduct and document thorough functional assessments using standardized procedures.
  • Develop and implement individualized treatment plans with achievable goals.
  • Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.
  • Instruct and supervise therapy assistants as needed.
  • Enter treatment and progress notes for each session.
  • Revise treatment plans as needed throughout the course of care.
  • Document treatment in clear, concise discharge summaries.
  • Orient residents to services and treatment procedures appropriate to their needs.
  • Collaborate with nursing staff, residents, and families to maximize functional abilities.
  • Educate residents and caregivers on home exercise and therapy continuation.
  • Assist with referrals for additional services as needed and maintain knowledge of community resources.
  • Participate in rehabilitation team conferences and other facility meetings.
  • Access continuing education opportunities to maintain clinical competency.
  • Participate in quality improvement initiatives.
  • Follow regulatory, professional, and facility standards.
  • Perform other duties as assigned to provide quality care.
  • Understand payer reimbursement and allocate therapy resources efficiently.
  • Support Ahava’s mission and project a professional image.
  • Maintain expected productivity levels.

About the Facility

Cordova Wellness & Rehab Center is a 242-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient therapy services.
