Job summary

Centerville Post Acute in Centerville, OH is seeking a licensed Physical Therapist to join our rehabilitation team. The PT will evaluate residents, create individualized plans, and deliver skilled therapy to improve mobility and independence.

Requirements include Ohio PT license and CPR; two years of post-acute experience is preferred. Excellent communication and teamwork are essential in our resident-centered care model.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the State of Ohio.
  • CPR certification required.

Responsibilities

  • Evaluate residents and develop individualized PT treatment plans.

We are looking for a team-oriented professional who is passionate about providing exceptional patient-centered care in a skilled nursing and rehabilitation setting.## Position SummaryThe Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing skilled physical therapy services that maximize functional independence and improve overall quality of life.
