OSR Physical Therapy, a local physician-owned private practice with six SW Metro locations, seeks a dedicated Physical Therapist. You will provide comprehensive outpatient PT care within a multidisciplinary team to achieve optimal patient outcomes.

Qualifications

• Graduate degree from a CAPTE-accredited Physical Therapy program.
• Current Minnesota state physical therapy license or eligibility.
• Valid CPR certification.
• Strong knowledge of anatomy, physiology, and rehabilitation principles.
• Excellent assessment and diagnostic skills.
• Ability to develop and implement effective treatment plans.

Responsibilities

• Conduct thorough patient evaluations and develop individualized treatment plans.
• Implement evidence-based physical therapy interventions and monitor progress.
• Document assessments, treatment plans, and progress notes with attention to detail.
• Supervise and coordinate care delivered by PT assistants and rehabilitation technicians.
• Establish measurable treatment goals and adjust plans based on progress.
• Educate patients and caregivers on plans and home exercise programs.
• Collaborate with referring physicians and other healthcare providers.
• Maintain accurate documentation in compliance with healthcare standards.
• Utilize rehabilitation equipment and techniques effectively.
• Participate in regular team meetings and care coordination discussions.
