Qualifications

• Graduate degree from a CAPTE-accredited Physical Therapy program.
• Current Minnesota state physical therapy license or eligibility.
• Valid CPR certification.
• Strong knowledge of anatomy, physiology, and rehabilitation principles.
• Excellent assessment and diagnostic skills.
• Ability to develop and implement effective treatment plans.

Responsibilities

• Conduct thorough patient evaluations and develop individualized treatment plans.
• Implement evidence-based physical therapy interventions and monitor progress.
• Document assessments, treatment plans, and progress notes with attention to detail.
• Supervise and coordinate care delivered by PT assistants and rehabilitation technicians.
• Establish measurable treatment goals and adjust plans based on progress.
• Educate patients and caregivers on plans and home exercise programs.
• Collaborate with referring physicians and other healthcare providers.
• Maintain accurate documentation in compliance with healthcare standards.
• Utilize rehabilitation equipment and techniques effectively.
• Participate in regular team meetings and care coordination discussions.

Additional Information

The salary range begins at $72,000 and increases based on relevant experience.Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for discretionary incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan
