Job summary

Eden Rehabilitation And Healthcare Center in Eden, NC is seeking a part-time Physical Therapist to provide comprehensive PT evaluations, develop treatment plans, and deliver skilled therapies to residents.

You will collaborate with an interdisciplinary team, document progress, ensure compliance with facility policies, and participate in staff education and meetings to support quality patient care.

Qualifications

• Graduate of an accredited university with a DPT or equivalent recognized by APTA.
• Holds current license in NC (as applicable)

Responsibilities

• Provide a comprehensive PT evaluation based on physician orders.
• Develop a comprehensive treatment plan with goals, frequency and duration.
• Obtain physician orders for evaluations, treatments and discharges.
• Document progress and discharge summaries in timely manner.
• Perform billing for PT services ethically and accurately.
• Supervise junior staff or students as needed.
• Participate in facility meetings and interdisciplinary team activities.
