Job summary

Windsor Rehabilitation And Healthcare Center in Windsor, NC seeks a Physical Therapist to join a dedicated team. The role offers full or part-time hours in a clinical setting with a multidisciplinary team, focused on quality patient care.

The position involves comprehensive evaluations, treatment planning, documentation, and collaboration with physicians to ensure optimal patient outcomes within facility policies.

Qualifications

• Graduate of an accredited university with a PT degree recognized by the APTA.
• Holds current license/registration in the state of practice.

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated while adhering to precautions.
• Document patient's daily participation and progress on weekly progress note accurately and timely.
• Document discharge and in-services on a discharge summary as indicated.
• Complete any and all other documentation forms required by Facility.

Education

PT degree (B.S./DPT/MA/MS) recognized by the APTA
