Job summary

Windsor Rehabilitation And Healthcare Center in Windsor, NC is seeking a Physical Therapist to join our long-term care team. The role offers full-time or part-time hours with a flexible schedule and competitive wages.

The PT will perform comprehensive evaluations, develop treatment plans with goals, document progress, and coordinate with the interdisciplinary team to provide quality patient care in a supportive environment.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy that is recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings and adhere to guidelines on standardized evaluation forms.
• Develop and implement a comprehensive treatment plan including goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver treatment using modalities and adjust plans as needed while adhering to precautions.
• Document daily participation and progress on weekly notes and update discharge summaries as appropriate.
• Attend facility and rehab meetings and participate in interdisciplinary team planning.
