Laurel Park Rehabilitation And Healthcare Center in Elizabeth City, NC, is hiring a Physical Therapist to join our rehab team in a long-term care setting. This position offers full-time or part-time hours and the chance to provide comprehensive PT services to residents.

Qualified candidates hold a PT degree, current license in the state, and strong evaluation and documentation skills. We offer benefits, Paid Time Off, and ongoing professional support.

Qualifications

• Graduate of an accredited university with a PT degree recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, accurately and adhering to all guidelines
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• Document patient's daily participation and progress on weekly progress note accurately and timely
• Document updated treatment plan in timely manner as indicated.
• Document patient's discharge and in-services on a discharge summary as indicated
