Responsibilities include comprehensive PT evaluations, developing treatment plans with goals, documenting progress, coordinating with physicians, and ensuring compliance with facility policies.

Qualifications

• Graduate degree in Physical Therapy recognized by the APTA.
• Current PT license/registration in the state.

Responsibilities

• Provide a comprehensive PT evaluation based on MD orders.
• Document findings on the standardized evaluation form adhering to guidelines.
• Provide a comprehensive treatment plan with long- and short-term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment using modalities and adjust as needed while adhering to precautions.
• Document daily participation and progress on weekly progress notes.
• Document updated treatment plan and discharge summaries as indicated.
• Complete all required documentation forms for the facility.
• Be involved in facility meetings and inter-disciplinary team activities.
• Perform any other duties assigned by supervisor or regional manager.
