Job summary

Laurel Park Rehab and Healthcare Center in Hertford, NC is seeking a part-time Physical Therapist to join a dedicated rehab team. The role focuses on comprehensive evaluations, development of individualized treatment plans, and close coordination with the interdisciplinary team to meet physician orders and patient goals.

Candidates must hold a PT degree and a current NC license; prior rehab experience is valued.

Qualifications

• PT degree completed from an accredited university (B.S., DPT, MA or MS) recognized by the APTA.

Responsibilities

• Provide comprehensive physical therapy evaluation based on MD orders.
• Document findings on standardized evaluation forms per guidelines.
• Develop comprehensive treatment plans with long- and short-term goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
