Laurel Park Rehabilitation And Healthcare Center in Elizabeth City, NC is hiring a Physical Therapist (PT) for full-time or part-time roles. The position requires a PT license, DPT/degree, and demonstrates excellence in evaluation, documentation, and interdisciplinary collaboration.

You will develop treatment plans, coordinate with MD orders, and ensure compliant care across evaluations, treatments, re-certifications, and discharges. Excellent patient care and teamwork are emphasized.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy that is recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings on the standardized evaluation form, accurately and adhering to all guidelines
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• Document patient’s daily participation and progress on weekly progress note accurately and timely
• Document updated treatment plan in timely manner as indicated.
• Document patient’s discharge and in-services on a discharge summary as indicated.
• Complete billing and bill patients ethically and accurately for physical therapy services rendered.
• Cover other therapists patient caseload during absence as necessary
