Laurel Park Rehabilitation And Healthcare Center in Elizabeth City, NC seeks a licensed Physical Therapist to join a dedicated rehab team. You will perform PT evaluations, create and adjust treatment plans, and coordinate with physicians to ensure proper orders and documentation.

The role emphasizes comprehensive patient care, adherence to facility policies, and active participation in interdisciplinary teams to support resident outcomes and quality of life.

Qualifications

• Graduate of an accredited university with a PT degree (B.S., DPT, MA or MS) in Physical Therapy.
• Holds current PT license/registration in the state of practice.

Responsibilities

• Provide a comprehensive PT evaluation based on MD orders.
• Develop and document a treatment plan with long and short-term goals.
• Ensure MD orders are obtained for evaluations, treatments and discharges.
• Deliver comprehensive treatment using modalities and adjust as needed.
• Document daily participation and weekly progress; update plan and discharge notes.
• Co-sign notes for junior staff and supervise students when required.
• Maintain confidentiality and adhere to facility policies and HIPAA/Resident Rights.
• Attend meetings and participate in interdisciplinary team planning.

Education

B.S./DPT in Physical Therapy

Current PT license in North Carolina or applicable state
