Job summary

Paycom - ATS is seeking a licensed Physical Therapist to provide and direct high-quality patient care in a long-term care setting in Sequim, WA. The role requires developing and implementing treatment plans, educating staff and payers, and ensuring documentation compliance.

The successful candidate will have a graduate degree in Physical Therapy, current licensure, CPR certification, and experience in geriatric care.

Qualifications

• Graduate of an approved Physical Therapy curriculum.
• Licensed Physical Therapist in good standing.
• CPR certification as required for license renewal.
• Experience in long-term care preferred.

Responsibilities

• Provide and direct all aspects of quality patient care according to guidelines and standards.
• Develop treatment plans and educate staff, other health care professionals, and payers on outcomes.
• Complete documentation including evaluations, progress notes, and monthly reports as required.
• Screen individuals who may benefit from therapeutic intervention.
• Coordinate with the rehab team and supervisor to implement new programs and improve utilization.
