Apploi is seeking a Physical Therapist to develop individualized therapy programs, perform assessments, and collaborate with health professionals to maximize a patient’s functional progress.

The role requires a PT license and a degree in Physical Therapy, with SNF experience preferred. You will document care plans, educate patients, and contribute to a positive facility environment.

Qualifications

• Graduate of an accredited PT program (Bachelors, Masters, or Doctorate).
• Current license from the state Board of Physical Therapy Examiners.
• Knowledge to care for SNF patient needs and regulatory standards.

Responsabilités

• Provides direct patient care, evaluates needs and outcomes, and adjusts plans.
• Designs and implements a care plan based on assessments and family involvement.
• Conducts therapy within defined standards of care.
• Directs support staff to help achieve patient goals.
• Communicates with other professionals to support positive outcomes.
• Follows facility policies, safety, and infection control standards.
• Documents evaluations, goals, and plans; maintains sign-offs.
• Pursues professional growth through in-service and workshops.
