Qualifications

• Graduate of an accredited PT program (Bachelors/Masters/Doctorate).
• Current license from the state Board of Physical Therapy Examiners.
• Proficiency with applicable state and federal standards.

Responsibilities

• Provide direct patient care and evaluate functional needs.
• Design and implement a plan of care based on assessment.
• Coordinate care with other health professionals.
• Direct support staff to achieve goals.
• Document evaluations, goals, and plans; maintain records and signatures.
• Participate in continuing education and stay current with literature.
