Job summary

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Minimum 1 year of experience in physical therapy, with a strong desire to work in a long-term care setting.

• Ability to lift, transfer, and support residents with varying weights and abilities.

Responsibilities

• Evaluate and treat residents with musculoskeletal, neurological, and cardiovascular conditions.
• Design and implement individualized treatment plans to promote resident independence, function, and mobility.
• Conduct daily evaluations and treatments, and provide education and training to residents, families, and caregivers.
• Collaborate with physicians, occupational therapists, and speech therapists to provide comprehensive care.
• Maintain accurate and timely documentation of treatment plans, progress notes, and resident data.
• Participate in care plan meetings and interdisciplinary team conferences.
• Develop and maintain effective working relationships with residents, families, and staff members.
• Stay current with best practices and standards of physical therapy practice.
• Participate in quality improvement initiatives and peer review processes.
