Qualifications

• Master's Degree in Physical Therapy is required.
• Current Physical Therapist Certification in NJ.
• 1+ years experience in Home Health, Acute Care, or Rehabilitation Facility.

Responsibilities

• Provide compassionate, patient-centered care in the home during visits.
• Conduct in-home assessments to evaluate mobility, function, pain, and safety.
• Develop individualized treatment plans to improve ADLs, strength, balance and independence.
• Deliver hands-on therapy in the patient’s home using evidence-based techniques.
• Educate patients and families on exercises, mobility techniques and safe practices.
• Track and evaluate patient outcomes and adjust plans of care as needed.
• Collaborate with interdisciplinary team to coordinate care.
• Document visits and progress in line with agency standards.
• Maintain reliable attendance to ensure continuity of care.
