Job summary

Mountainside Nursing Home seeks a dedicated Physical Therapist to join our team. You will evaluate and treat residents, design individualized plans to promote independence and mobility, and collaborate with physicians, therapists, and care staff to provide comprehensive care.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Minimum 1 year of experience in physical therapy.
• Current licensure in the state as a Physical Therapist.
• Ability to lift, transfer and support residents with varying weights.

Responsibilities

• Evaluate and treat residents with musculoskeletal, neurological, and cardiovascular conditions.

We are seeking a dedicated and compassionate Physical Therapist to join our team at Mountainside Nursing Home, where we are committed to providing exceptional care and services to our residents. As aPhysical Therapist, you will be an integral part of our interdisciplinary team, playing a vital role in helping our residents achieve their highest level of physical function and independence.

About the Role:

As a Physical Therapist at Mountainside Nursing Home, you will be responsible for:

• Evaluating and treating residents with a variety of physical impairments, including those with musculoskeletal, neurological, and cardiovascular conditions
• Designing and implementing individualized treatment plans to promote resident independence, function, and mobility
• Conducting daily evaluations and treatments, as well as providing education and training to residents, family members, and caregivers
• Collaborating with other healthcare professionals, including physicians, occupational therapists, and speech therapists, to provide comprehensive care
• Maintaining accurate and timely documentation of treatment plans, progress notes, and resident data

Additional Responsibilities:

• Participate in care plan meetings and interdisciplinary team conferences
• Develop and maintain effective working relationships with residents, families, and staff members

Requirements:
