Job summary

Mountainside Nursing Home seeks a dedicated Physical Therapist to join our team. You will evaluate and treat residents, design individualized plans to promote independence and mobility, and collaborate with physicians, therapists, and care staff to provide comprehensive care.

You will maintain documentation, participate in care plan meetings, and stay current with best practices to improve resident outcomes.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Minimum 1 year of experience in physical therapy.
• Current licensure in the state as a Physical Therapist.
• Ability to lift, transfer and support residents with varying weights.

Responsibilities

• Evaluate and treat residents with musculoskeletal, neurological, and cardiovascular conditions.
• Design and implement individualized treatment plans to promote independence and mobility.
• Conduct daily evaluations and treatments and provide education to residents and families.
• Collaborate with physicians, occupational and speech therapists for comprehensive care.
• Maintain accurate documentation of plans, progress notes, and resident data.
