Job summary

Mountainside Nursing Home in New Jersey is seeking a dedicated Physical Therapist to join our interdisciplinary team, helping residents achieve their highest level of physical function and independence.

You will evaluate and treat residents with musculoskeletal, neurological, and cardiovascular conditions, design individualized plans, and collaborate with physicians, therapists, and family members while maintaining detailed documentation and participating in care plan meetings.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Minimum 1 year of experience in physical therapy, with a strong desire to work in a long-term care setting.
• Current licensure in the state of New Jersey as a Physical Therapist

Responsibilities

• Evaluate and treat residents with physical impairments including MSK, neurological, and cardiovascular conditions.
• Design and implement individualized treatment plans to promote independence, function, and mobility.
• Conduct daily evaluations and treatments, and provide education to residents, families, and caregivers.
• Collaborate with physicians, occupational therapists, and speech therapists to deliver comprehensive care.
• Maintain accurate and timely documentation of treatment plans and progress notes.
• Participate in care plan meetings and interdisciplinary team conferences.
• Develop and maintain relationships with residents, families, and staff.
• Stay current with best practices and standards of physical therapy practice.
• Participate in quality improvement initiatives and peer review processes

About the Role:

As a Physical Therapist at Mountainside Nursing Home, you will be responsible for:
