Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide Hospice therapy in residents' homes in Auburndale, FL. The role emphasizes compassionate, evidence-based care to help older adults maintain independence and quality of life.

This PRN position offers flexible assignments, no required commuting between homes, and competitive pay. You will evaluate and treat residents in an assisted living or home setting, develop treatment plans, and document in the EMR.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR
