Job summary

Sobe Innovative Rehab seeks a licensed Physical Therapist to provide hospice therapy in a home setting in Kissimmee, FL. You will work PRN to support older adults in maintaining independence and quality of life through evidence-based care.

Qualifications

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
